At First go to the official website and click here https://pmaymis.gov.in/ and In the second option, click on Citizen Assessment then click "benefit under other components" .
then open new page Check Aadhaar/Virtual ID No. Then type in the box Aadhar no and Name as per Aadhaar, check box right click then click check.

open new page Format B - Required Information of Survey from Enter all your information correctly here Name,address ,Aadhar number ,employee type family member ,family member details, ration card, bank account all details with IFSC, etc.
Then read disclaimer and click the checkbox then type captcha image then finally save & submit your details .
You will receive your application number as soon as you submit. You can check the status with that application number. Leave that application number with you.
